Protocol for Identity Checks
The Board's process for identity checks for persons wishing to apply for
registration as an optometrist in Tasmania are as follows:
- Application for registration forms
should be lodged directly with the Board office and include an
original passport size photograph of the applicant and the
appropriate fee.
- All applicants are also required to
present in person, either at the Board office or to the Board’s
authorised representatives in the North and North West of the state.
- Applicants must present with a
second original passport size photo, plus photo identification such
as a current drivers licence or passport.

National Police
History Check
The Board requires a full current
National Police History Check to be submitted with all
applications for registration. For further information and to obtain
an application form, contact your local police station (within
Australia). The fee for the check is borne by the applicant.
1. Registration under the Optometrists Registration Act 1994
A graduate of an
institution accredited by the Optometry Council of Australia and New Zealand
(such as Australian and New Zealand schools of optometry),
who is not currently registered in another state or territory of
Australia or New Zealand, may apply for registration as an
optometrist in Tasmania under the Optometrists’ Registration Act
1994 (the Act).
In addition to
having an accredited qualification, the Act specifies that a person
applying for registration must satisfy the Optometrists Registration
Board that they:

have
sufficient physical capacity, mental capacity and skill to
practise optometry

are of good
fame and character
-
have an
adequate command of the English language
-
are an
Australian citizen or reside in a state or territory of the
Commonwealth.
